Most travel companies have a medical officer who can help you decide whether it’s safe and practical for you to travel. The medical officer can arrange things like early boarding on the plane/ferry/coach and wheelchair access if it’s needed.

When you’re booking your holiday, it’s a good idea to let them know about any disability you might have so that this can be taken into account. If your doctors think it advisable, you might want to take a summary of your medical condition with you (just in case any problems crop up when you’re away).
